Type II – Covered 80% Example - Fillings, Extractions, Root Scaling, Root Canal

Orthodontics - Per person lifetime maximum of $2,500.00

Davis Vision
The Optical Assistance Program is offered through Davis Vision/Vision Works. When you see a participating Davis Vision provider you are eligible for the following:
· Eye Examinations every 12 months
· Eyeglasses or Contact lenses every 24 months.
· Every 12 months for dependents up to age 19
